Special Events Intake Form

  1.  Download the Special Event Toolkit and Operations Plan form.
  2. Open the document and enable macros and content control, if prompted.
  3. Click the hyperlinks on the form to assist you in knowing policies and procedures regarding Special Events management within the Student Centers and associated facilities.
  4. Fill out the form based on the information that you have regarding your Special Event.
  5. Once completed, you may submit the form by:
    1. Submit the form directly using Microsoft Outlook and Microsoft Word 10 and up, by clicking the "Submit" button at the end of the form.  Or...
    2. Attach a completed copy to an email and send to scasched@hartford.edu.  In the subject line, please put "Operations Plan Submission.
