Division of Student Affairs Student Leadership Nomination Form

Please use one form per each nomination. Nomination deadline is, Wednesday April 6 at noon.  If you nominate an individual, or you are the winner of an award, you will receive notification inviting you to the ceremony.

Mark your calendar:  the 2016 ceremony will be held Sunday, April 24  at 10 a.m. in Gengras Student Union Cafe.
