Campus Visitor Form
In an effort to keep our community safe, access to campus will be largely limited to current students, faculty, and staff.
There may be some cases in which immediate family members or approved vendors need to come to campus. In those instances, they will be required to submit this request form at least 24 hours in advance. See our campus access policy and residential life guest policy for more details.

Employees and current students do not need to complete this form.
Guests will an email from Public Safety with the status of their request. Please be prepared to show this email approval at the front gate for entrance onto campus.
LiveSafe Daily Screening
All faculty, staff, students, and approved guests need to complete a health screening on LiveSafe before coming to campus or attending class. Approved visitors should download our LiveSafe application and complete the screening the day intended to arrive on campus.
Guests will be asked to show the green checkmark on the health screening upon entrance.
