Submit Vaccination Information for a Chance to Win
We encourage everyone in our community to get vaccinated against COVID-19 to protect themselves and each other. A strong vaccination rate on campus will also allow us to continue planning for a less restrictive fall semester.
To help us make these decisions, once you are fully vaccinated, please log on to the Student Health Portal using your UHart email credentials:
- Click “My Forms”
- Click on “COVID-19 Vaccine”
- Enter the dates of any vaccinations you have received
- Upload an image of your CDC Vaccination Card
If you submit your information by Friday, May 7, you will be entered to win one several prizes, including:
- Keurig Single Cup Coffee Maker
- Kodak Portable Instant Photo Printer
- Bose Bluetooth Speaker
- Portable charger
Winners will be chosen on May 7.
