As a program licensed by the State of Connecticut Office of Early Childhood, Summer Place requires a Physician's Medical Record on file for every camper. The medical exam must be dated after August 8, 2022. Medical records are not kept on file year to year, so a new form is required each year. Parents/guardians should complete page 1 of the medical form. The child’s physician completes Page 2.
Forms cannot be uploaded through the registration site and must be emailed to splace@hartford.edu.
Campers may not attend unless all required medical forms are received by May 1, 2026.
If your child will take medication at camp (prescription or non-prescription), additional authorization is required. Medication must be provided in its original container with the prescription label intact. The camp nurse and staff must be informed of any medical needs. Parents may request to speak with the nurse before camp begins.
The State of Connecticut also requires an Individualized Plan of Care for any camper with a health condition or disability requiring special consideration. These plans are sent by email and must be signed and returned before attendance.
